Summary:
|
San Bernardino County, through Arrowhead Medical Center, operates a schoolÂ-based mobile asthma treatment program referred to as "Breathmobile Clinic" and will operate the Breathmobile Clinic on district property. The services by the Medical Center will be on the school sites on days and times that are mutually agreed upon by the district and medical center. Arrowhead Medical Center will be solely responsible for staffing and providing services to the students. The staff and/or trainees providing the services are trained and prepared according to professional standards for providing such services. They will provide adequate supervision of the staff and/or trainees. Services provided will be done with parent/guardian written approval.
Services will include:
* History and physical examination
* Comprehensive Primary Medical Services
* Referrals for additional care where indicated
Obligations of the district/school site are as follows:
* Provide the Breathmobile Clinic with necessary utilities and restroom facilities for clinic staff
* The District Nurse staff will identify students who are high-risk asthma patients
* Coordinate patients/parents for scheduled Breathmobile visits
* Monitor asthma status utilizing individual treatment plans
* Notify Breathmobile Clinic regarding patient status as necessary |

Funding:

|
Services will be provided at no cost to the district or to the students served. The Medical Center will bill Medi-Cal and other third party payers for eligible service. |

Recommendation
|
|
It is recommended that the Governing Board of Trustees approve the agreement between the County of San Bernardino on behalf of Arrowhead Regional Medical Center and Silver Valley Unified School District as presented. |
